Your application doesn't necessarily have to follow this template, but I would like you to at least include all of the information asked for in this template in your application. Of course, your app can follow this template if you want it to.
Remember, create a new account for each character you are RPing, and make sure your display name matches the name of your character. For example, if I'm RPing Severus Snape, I would switch accounts to my Severus Snape account.
